Yes, IBD qualifies as a disability that can qualify you for a 504 plan in any K-12 school that receives federal funding. 504 plans are intended to allow for equal academic access, so can provide accommodations like extensions for assignments missed due to medical conditions/appointments, preferential classroom seating, unlimited bathroom access, and/or extra time on tests.
